Community Medical Respite Program
Homeless men and women suffering acute medical conditions can be referred to our Community Medical Respite Program. This program specifically serves those medically fragile individuals who need consistent medical care but who are not ill enough to stay in the hospital. Individuals receive the medical care and social services they need with an ultimate goal of placing them in permanent housing. In collaboration with other agencies, we provide the following services through the Community Medical Respite Program:
- Temporary shelter
- Case management
- Oversight of medicines
- Medical care by social workers and nursing professionals
Patients in the Community Medical Respite Program are by referral only. If you are a medical professional or social worker and have a patient whom you believe fits the criteria of our program, please contact the Community Medical Respite Director, at (919) 828-9014, ext. 133.
